The following is registration information for expectant parents who wish to donate their child’s cord blood. All completed forms must be received by our office prior to the 34th week of pregnancy so that they can be screened by the Medical Review Panel and to ensure that the cord blood collection kit is received prior to birth.
NOTE: You are not a candidate for our program if you he ever tested positive for
HIV or are a carrier of Hepatitis B
You must register before your 34th week of pregnancy.
STEP 1: COMPLETE THE FORMS
Print the PDF Registration forms. Both Mother and Father must complete and sign all forms.
SPECIAL NOTE: These forms are only for people interested in DONATING cord blood to Victoria Angel Registry of Hope.
STEP 2: RETURN COMPLETED FORMS TO THE REGISTRY

STEP 3: AWAIT CONTACT FROM REGISTRY
Once all the paperwork is received, a representative will contact you to discuss the process for donating cord blood.
