4 STEP INSTRUCTIONS

STEP 1: REVIEW THE REFERENCE COPY OF THE AGREEMENT
The Agreement is written to help parents understand the process of cord blood collection and storage. It outlines the responsibilities of both Cells for Life and the parents. Pricing information is included in Exhibit A of the agreement. Note that the attached copy of the Agreement is a reference copy for your review; the original copy is included in your Collection Kit.   Cells for Life offers financing plans and savings for returning clients or those parents expecting twins.

NOTE:  You are not a candidate for our program if you have ever tested positive for HIV or are a carrier of Hepatitis B.

STEP 2: ACQUIRE A CORD BLOOD COLLECTION KIT PRIOR TO THE                BIRTH OF YOUR BABY
Our kits have been specifically designed to ensure the safe collection and transport of your baby’s cord blood. All instructions are easy for parents to understand. Parents can obtain a kit by:
           
        A. Courier: We can ship the kit directly to you (fees apply). This
          can be easily arranged by calling us at 905 472 0060 ext 29 or           by emailing us at shipping@cellsforlife.com Our Shipping           Coordinators will call you to organize the safe delivery of your           kit.

        B. Attend a Group Education Session:   Cells for Life provides           educational information sessions to parents and health care           workers free of charge at several locations. Please call           905 472 0060 ext 0 or info@cellsforlife.com for more           information. Collection kits can be picked up at these sessions.           It is a great opportunity to have your questions answered.
           
        C. Pick up at our offices:  Kits can be picked up at our Cells for Life           offices. We recommend making an appointment so that our staff           can review the kit’s contents and procedures:

If you are unable to attend a group education session and wish to learn more about cord blood banking and Cells for Life’s procedures, you may book a private education session with our Registered Nurses.  

STEP 3:  REVIEW THE PARENT INFORMATION PACKET IN THE KIT AND                 COMPLETE ALL NECESSARY FORMS PRIOR TO YOUR DUE                 DATE.
All registration forms, payment forms, and contracts are included in your collection kit. These forms must be completed and returned to the laboratory with your cord blood sample.

STEP 4:  RETURN YOUR BABY’S CORD BLOOD TO CELLS FOR LIFE AFTER                 THE BIRTH
The kit must be returned as soon as possible after your baby’s delivery. The shipping process is easy --- all instructions are included in the Collection Kit. Parents have three options for returning the kit to our laboratory.

        A. Deliver it yourself:  You can drop off your own kit or organize a           friend/ family member to deliver it to our Cells for Life office at           the Markham Stouffville Health Centre or at the Toronto General           Hospital. Please note times when the office will accept deliveries           (this is clearly written in your kit instructions.)

        B. Cells for Life Bedside Pickup: We are pleased to offer bedside           pickup service to clients delivering in selected Toronto area           hospitals. (Fees apply)  Please enquire at registration if you are           interested in this program. Information is also included in your           collection kit.

        C. Use a Courier Company:  Shipping instructions are included in           your collection kit; names of experienced couriers are included           also. Our shipping department can answer any questions so that           pick up is worry-free.
