(to the left). All completed forms must be received by our office prior to the 34th week of pregnancy so that they can be screened by the Medical Review Panel and to ensure that the collection is kit is received prior to birth.
STEP 2: Return Completed Forms to Registry
Once all of the forms have been completed and signed by both the Mother and Father, please fax them to Victoria Angel Registry of Hope at

STEP 3: Await Contact from Registry
Once all paperwork is received, a representative of the Victoria Angel Registry of Hope will contact you to discuss the process of donating cord blood. Thank you in advance for considering donating your child’s cord blood for others to use. It is truly a Gift of a Lifetime.
